| 1. | Provider must be at least 18 years of age and have a high school diploma or GED. |
| 2. | Provider must complete a physical examination and have required immunizations. |
| 3. | Provider must have a follow up physical examination every three years. |
| 4. | Provider shall be physically, intellectually, and emotionally capable of performing activities related to child care. These include providing meals, dealing with emergencies in a calm manner, carrying out child discipline in a controlled and patient manner, and keeping accurate records for billing purposes. |
| 5. | The Provider must have at least six months experience in caring for a child 12 years or younger, or 30 hours of documented child related training. |
| 6. | The Provider must provide a safe, healthy environment for children and not participate in activities or behaviors that endanger the health, safety, and well being of children. |
| 7. | The Provider and any person that resides in the home will provide background checks to ensure that they have no history of violence, drug use, or demonstrating physical or mental conditions that could be potentially harmful to children. |
| 8. | Each Provider must provide at least three written references from individuals unrelated to the applicant, who can provide information related to the applicant's ability to provide care for children. |
